Check, below, the required documents to be presented upon surgical admission:

• Admission Form
• Pre-operative Medical Prescription (when applicable)
• Document of the patient with photo
• Healthcare plan card
• Prior test results (if necessary)

Informed Consent Forms (previously signed by the physician and patient in the doctor’s office)

• Informed Consent Form (ICF) for related and unrelated allogeneic bone marrow transplant
• Informed Consent Form (ICF) for donation of hematopoietic stem cells (autologous transplant)
• Informed Consent Form (ICF) for hepatic transplant with liver from extended criteria donor
• Informed Consent Form (ICF) for liver donation for transplant
• Informed Consent Form (ICF) for risks of liver transplant (for recipient)
• Informed Consent Form (ICF) for kidney donation for transplant
• Informed Consent Form (ICF) for risks of renal transplant (for recipient)
• Informed Consent Form (ICF) for blood and/or blood product transfusion
• Informed Consent Form (ICF) for risk of admission and procedures at the Intensive Care Unit (ICU) and/or Stepdown Unit
• Anesthesia and Sedation
• Surgical or Invasive Procedure
• Informed Consent Form (ICF) for donation of hematopoietic stem cells for transplant
